Part of your problem is that you have two copies of the database loaded:

It's at that point that it loads the second copy, so something about freshclam is likely not seeing the database that clamd is loading, downloading a second copy, clamd is HUP'd and loads the second copy. Probably some difference in configuration, but I've not wrapped my head around it. My fix (a while back) was just delete /var/clamav/*, run freshclam manually, start clamd, then start freshclam daemon. No problems since then.
